Apply Online
爱豆传媒 uses the statewide application system known as OpenCCC. This online system is designed to accommodate most students.
Complete the 爱豆传媒 College application and you will receive your CampusWide IDentification (CWID) number and password to access the college鈥檚 .
TIP
When you fill out the application, remember to declare a major (not "undecided") and select a goal of transfer, degree or certificate if you want to qualify for priority enrollment.
Submit DSPS Application
Complete and submit the online DSPS Application for services:
- Login to MyPortal,
- Select the Student tab and find the 鈥ClockWork Student Portlet鈥, and
- Select 鈥ClockWork 爱豆传媒 Disability Progs & Svcs鈥.
Submit Disability Verification Form
Submit a completed Disability Verification Form to verify your disability(ies):

Give your Doctor or appropriate health professional the:
- Instruction Letter, (NOTE: Fill in the date)
- Disability Verification Form (NOTE: You must fill out the Student Information portion before giving the form to your Doctor or appropriate health professional).
- Disabilities Definitions and Documentation sheet.
Return the completed Disability Verification Form to DSPS.
Submit Options:
- FAX to (408) 864-5492
- Have it scanned then e-mail as an attachment to DSS@fhda.edu
- Bring it in person to RSS 141
- Scan it before step 2 then upload it during the online DSPS Application process.

Complete these steps and you will be promptly scheduled to meet with a DSPS Counselor to set up all of your classes, services, and accommodations.
